Our Services

At Patient Centered Care, we care about our patients and are here to make life easier for you and your loved ones. Our wide range of home care services are designed to assist all of your health needs. From Private Duty to 24-Hour Care — our dedicated staff work tirelessly at improving the quality of life for each and every one of our patients and their families.

Personal Care

Personal Care Services (PCS) provides medically necessary assistance with activities of daily living (ADL) and age appropriate instrumental activities of daily living (IADL) that enable the recipient to accomplish tasks that they would normally be able to do for themselves if they did not have a medical condition or disability. Assistance with ADLs consist of eating, bathing, dressing, toileting, transferring and maintaining continence. IADLs consist of personal hygiene, light housework, meal preparation, grocery shopping, as well as activities and appointments.

24 Hour Care

24 Hour Care provides various services, including personal care, light housekeeping, meal preparation, medication reminders and companionship. Two caregivers are booked daily and work 12 hours each. Sometimes patients prefer to book three caregivers and each work 8 hour visits in order to ensure caregivers are at their maximum level of alertness. The benefit of having 24 hour care is that a caregiver will be with a patient at all times.

Respite Care

Respite Care (RC) provides temporary relief for a primary caregiver, enabling you to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member. Respite care takes place in the patient’s home and is a separate service from home health, personal care, and/or palliative/hospice care. Whether it’s for just a few hours a week or an extended vacation, seeking respite care can help ease the burden of family caregiving and help to relieve stress, restore your energy, and promote balance in your life. It can also prevent you from becoming exhausted, isolated, or even burned out. Respite care can benefit the person you’re caring for, too, providing them with variety, stimulation, and a welcome change of routine.

Private Duty

Private Duty (PD) is home care that is delivered on a private-pay basis, which means patients or their families pay e the home care agency for services provided.  Because the service works on an individual basis, private duty home care services can be customized to meet the unique needs of each patient. Patients who hire us for private duty have the ability to access as much support as they feel they need, without seeking approval from their primary care physician or any other medical professional. With private duty care, there are no eligibility requirements, and patients are free to use the service on a short-term or long-term basis.
 

Companion

Companion Care (CC) is a type of in-home care service that provides essential assistance to the elderly and disabled patients. Care will typically include a number of ways to make daily living easier and safer for the care recipient. For example, a caregiver might perform errands, such as grocery shopping or picking up medications, on behalf of the patient.  At other times, caregivers will plan and prepare regular meals, perform light housekeeping duties, and offer medication reminders. This level of care is also designed to improve the patients quality of life in other ways. Caregivers act as partners for favorite hobbies and activities, accompany clients to appointments and social outings, and engage in friendly conversation with clients. These activities can be a lifeline for socially isolated seniors. Social isolation has shown to dramatically increase the risk of health complications and mortality in seniors. Companion caregiving provides isolated seniors with social interaction and regular companionship, ending the cycle of senior isolation.
